CONSENT FORM
​SSDCA Music Video - Sunday 5th March 2023

As you will already be aware, Simon Says Dance Commercial Academy students have the opportunity to be part of a Music Video, filmed in Edinburgh on Sun 5th March 2023.  In order to publish and share this media (when finished), we need the permission of a parent/guardian for children under the age of 16. Any children aged 16+ may submit their own permission form, but parents must mutually agree on taking part/sharing of media. 

Session2.com is our official videographer for the Music Video and will collate and edit the media for sharing - they will have access/store the footage for SSD. Simon Says Dance will also have access, store and share the Music Video on their relevant social media channels and website.
