PHOTOSHOOT CONSENT FORM

To take part in our upcoming marketing update photography shoot, we require all parents/guardians of children participating to complete the below consent form. Note that photography taken by Ric Brannan Photography will be used by Simon Says Dance for promotional purposes, including but not limited to social media, website and any other print or online platform. Ric Brannan Photography may also use photography for their promotional purposes. Images will be stored by Simon Says Dance Ltd and Ric Brannan Photography for the purpose of usage.
