CONSENT FORM
​SSDCA Music Video - Sunday 7th June 2026

As you will already be aware, Simon Says Dance Commercial Academy students have the opportunity to be part of a Music Video, filmed on Sun 7th June 2026 at the location shared by email. In order to publish and share this media (when finished), we need the permission of a parent/guardian for children under the age of 16. Any children aged 16+ may submit their own permission form, but parents must mutually agree to taking part/sharing of media. 

This music video is being filmed by Session2.com/Performance Filming Edinburgh. Collectively, the videographer (Session2.com/Performance Filming Edinburgh), the venue facilitators (Hidden Door Arts) and music video co-ordinators (Simon Says Dance Ltd) reserve the right to share any completed material on their website/respective social media platforms for marketing purposes.
